1. You've just arrived in France or haven't been to a doctor yet
Go to :
www.ameli.fr
On the homepage, click on « assurés ».
Then, enter your zip code in the green square at the top of the page. Then click on « modifier ».
Click on « votre caisse » (blue below) and on « nous
rencontrer ». You will see the address of the different offices.
Go to the closest and ask for the forms :
« Bonjour, je voudrais devenir assurés social »
« Bonjour, je voudrais les formulaires pour devenir assuré social ».
If you only need information, just ask :
« Bonjour je voudrais des renseignements ».
If this office is not the right, it's not a problem, they might give you the forms you need, the address of « your » « centre de paiement » and all you need.
2. You already have your carte vitale
The address of your « centre de paiement » is written on the reimbursement letters « sécurité sociale » sends you when each time you go to a doctor and it's also written on the letter you received with your « carte vitale ». You should always keep the last reimbursement letter in order to check all is ok with your reimbursement and also to have the address of your « centre de paiment » (it's not written on your « carte vitale »).
