You must fill in the upper part of the sheet : « personne recevant les soins et assuré(e) »

A. « Personne recevant les soins » : patient

1. "nom et prénom" : surname and first name

2. "numéro d'immatriculation" : number on your « carte vitale »

B. "Assuré (à remplir si la personne recevant les soins n'est pas l'assurée)" : fill this in if you are the patient but the carte vitale is not in your name. In this part, you must indicate the name and address of the "assuré social".

1. "nom et prénom"

2. "numéro d'immatriculation"

C. "Adresse de l'assuré" : address of the "assuré social"


So, if you are the "assuré social", fill in out only A & C.
If you are not the "assuré social" (but a spouse, a child), fill in out A, B and C.
