But what can I do if I've lost these documents ?
If you've lost them you can find the address of your “centre” on the web.
Go to :
ameli.fr
On the homepage, click on « assurés ».
Enter your zip code « code postal » in the green square on top of the screen. Click on « modifier ».
Click on « votre caisse » (blue at the bottom) then on « nous rencontrer ».
On the screen you'll see different addresses of « centres de paiement », choose the closest to your home and send your « demande d'entente préalable ».
If you're still not sure it's the right « centre », ask them, they will tell you which one is yours.
